FEES:

Initial Consult- $65. Session is 20 minutes. Please arrive 15 minutes prior to your appointment time with your intake packet filled out (download below) so we can go over them along with any additional forms needed. During this session, we will discuss your health concerns and make treatment recommendations as necessary. Referrals may be made to specialists when indicated.

Below are some of the services that we may recommend following your Initial Consult:

• Comprehensive Medical History & Physical Exam - $125. Session is 60 minutes.
• Basic Lab Fee - Lab includes comprehensive blood, urine, and stool tests.
• Additional diagnostic lab services may include tests for - Hormones, vitamins, minerals, allergies, toxic heavy metals, nutritional genetics, and gastric acidity.
• Follow-up appointments - $125 per hour. Session lengths vary.
• Phone consultations - $125 per hour. Consultation lengths vary.

BILLING:
We do not bill patients nor insurance. Payment is expected at the time of the visit - cash, check, visa and mastercard are all accepted. For phone consultations, we will ask for your credit card information to keep on file.

INSURANCE:
Some health insurance companies cover naturopathic physicians. However, we do not bill nor accept payment from health insurance companies. We will provide you with a "superbill" that you may send to your insurance company and they may reimburse you directly. If you would like to know if your insurance company will cover any of the visits and lab work from Blackbird Clinic PLLC, we recommend that you call them before your initial visit. We are not DSHS providers and are not covered by either Medicaid or Medicare.

APPOINTMENTS:
Patients are seen on Tuesdays, Wednesdays and Thursdays.

Initial Appointment: Please bring your new patient intake packet (download above) and arrive 15 minutes prior to your appointment to complete additional forms.

CANCELLATION POLICY :
Please give a minimum of 48 hours notice if you need to reschedule. $45 for missed appointments.

From the North: Take highway 97 south. Take first Okanogan exit, crossing the bridge over the Okanogan river. Turn left at the flashing red stop light onto Main street. Go one block and turn right on Pine Street. Blackbird Clinic is a one story brick building on the right side of the street between the Okanogan Public Library and Baines Title Insurance. Parking is on the street.

From the South: Take highway 97 north. Take the first Okanogan exit crossing the bridge over the Okanogan river. At the stop sign turn right onto the main street and drive into Okanogan. Turn left at Pine street which is one block before the flashing red stop light. Blackbird Clinic is a one story brick building on the right side of the street between the Okanogan Public Library and Baines Title Insurance. Parking is one the street.

From the Methow Valley: Take highway 20 to Okanogan. Turn left at Pine Street which is one block before the flashing red stop light. Blackbird Clinic is a one story brick building on the right side of the street between the Okanogan Public Library and Baines Title Insurance. Parking is on the street.
